Insurance Policy No., Participation No., Account No., etc.?
I am filling out a HIPAA Authorization For Release of Protected Health Information from Assurant Employee Benefits and the basics ask for “Policy No.” “Participation No.” “Account No.” and “Certificate No..” Where do I find this information? It also asks for the “Insured/Member name” but I’m not sure which name to put; I’m put down on someones work insurance so is it my name or the person who put me down on their work insurance?
The “Policy No.” “Participation No.” “Account No.” and “Certificate No..” should be on the health insurance ID card. If your not sure turn the card over and call the customer service number that is shown on the back of the card.
The “Insured/Member name” is the person who put you down on their work insurance.
