What should a letter say giving my sister permission to take my son to the doctor?

My son has an appointment with a specialist tomorrow and I’m not able to take him so my sister is taking him for me. I need to send a letter with my sister granting her permission to discuss the problem and possible treatment to avoid a big HIPAA fuss. I’m just not exactly sure how to word it. Any advice from health care professionals would be greatly appreciated, however I’m open to anything. Thanks.
