how do they not violate hipaa privacy laws when filming shows like, “trama life in the ER”?

I’m guessing they have to ask them for permission to film them and use their name do they ask them before or after because a lot of the time it would seem impossible to ask that question to someone that just came in to the ER with a bullet in his mouth making it hard for him to speak. So what do they do just film it before they have permission and then ask them when they are more stable? also do the patients get paid for their stories?

Yes and yes.

Anyone they can’t get clearance from, they’ll blur faces and bleep details so that they aren’t personally identifiable. Also, the show hasn’t been made in about ten years, so more recent laws protecting patient privacy didn’t apply to them.
