HIPAA – what do you need in order to use images of pts. on a “testimonial” section on a private prac website?
I was just wondering what kind of consent or form I would need to have patients sign in order to use their images on my website (private practice optometry). I’d like to have a testimonial section where they describe their experience and success and then possibly use their picture as well.

As long as you have their signed permission you should be okay.
There’s not a standard HIPAA approved form for that kind of purpose.
