Do I need to disclose a “diagnosis” or “illness” when giving my employer a doctor’s excuse. Is this a violatio?

When calling in for work my employer requires that I have to have a doctor’s excuse with a Md or NP, PA’s signature. The doctors excuse also has to state my illness and why I wasn’t able to perform my job duties. If I don’t provide this information my employer states that this would result in a disciplinary action. My doctor insists that this is a violation of hipaa and all he will put on a doctor’s excuse is that he has seen me on that date and along with his signature and phone number for any questions. Is this a violation of hipaa and do I have the right to make an issue of this matter?
Does anyone have the actual HIPPA law that I can print ? My employer won’t beleive me unless i actually bring it to his face

The signed statement of a physician should be sufficient evidence for one to present to an employer to excuse him/her from work. The doctor is liable if the employer can prove that the doctor made false declaration on the certification that was issued to the patient. No need for diagnosis of illness which is just between the physician and the patient.
